This form must be completed on a weekly basis for drop-in programming. Participants and volunteers must sign up for a session at least two days in advance so we can plan accordingly. Participants with and without disabilities are welcome!
Participants and volunteers must complete and submit an Adaptive Sports & Fitness Waiver prior to attending programming – send completed waivers to [email protected]
